1.Warm up: Play Games: Lucky numbers
Give the past form of the verb “have” Make sentence with “school” Answer: “Where do you go when you have a toothache?” Make the question: “I’m 1m45 tall.” Lucky number. Answer: “What time do you go to bed?” 2. Presentation: T explains the request. Ss read model sentences to know the information. 3.Practice: Have Ss work in pairs and cover the form. Some pairs ask and answer before the class. Complete the form. 4. Production: Ss ask/answer and fill their own information in their medical records. 5. Homework: copy the medical record.
